Client Agreement Form

1. I understand that Melissa Navarro & Julien Griffault of healingaia Holistic Wellness do not present themselves as medical doctors nor as possessing formal western medical training. I understand that healingaia services and products are not substitutes for medical treatment, procedures or prescription medication recommended by my healthcare professional.

2. I agree to provide current, complete and accurate information about myself as required for each particular session/service.

3. No one representing healingaia Holistic Wellness offers me any false hope, false promises, expectations, warranties or assurances of the success or the outcome of any of healingaia's work.

4. I have read and understand healingaia's fees and that they are pre-paid BEFORE my appointment or class, with the exception of Bodywork services.

5. I understand and agree to the following: a) I need to give healingaia 24 hours notice to cancel or re-schedule my appointment b) If I miss my appointment without giving healingaia 24 hours notice, I will am responsible for 50% of the full service price c) I am responsible and I pay for all phone charges incurred, if any, for all phone consultations with healingaia.

6. If I purchased a Home Cleansing, Protection & Blessing, I agree to pre-pay $333 for the first 2 hours of service and $106 per hour for any additional working time. I understand and agree that I will receive a PayPal invoice following completion service for any additional working time.

7. I am 18 years of age or older, of sound mind and not under the influence of any mind altering drugs. By clicking "I HAVE READ AND AGREE" I am electronically signing this document and I acknowledge that I read the above, have thoroughly reviewed and understand its content and that I am giving my informed consent and it is my intent to agree to this contract. By my electronic signature I understand that this document becomes a legally binding contract and is confidential. This contract shall be governed by and construed in accordance with the laws of the state of New Jersey.

8. Consent by Legal Guardian, Parent or Attorney In Fact: As the parent and or Legal Guardian, or POA, I acknowledge that I have read the above, have thoroughly reviewed and understand its contents, and that I am giving my informed consent. By clicking "I HAVE READ AND AGREE" I am electronically signing this document and I hereby authorize healingaia Holistic Wellness to provide the purchased services for the person/pet I am responsible for.

1 Healingaia and its Logo are proprietary marks owned by Healingaia, LLC. All other copyrights, product names, trade names, trademarks, slogans, and/or designs depicted in the Healingaia web site are the proprietary items of their respective owners.

The statements made about products and services have not been evaluated by the U.S. Food and Drug Administration, therefore any results reported, if they exist, may not necessarily occur in all individuals.
